The source is here.
The original subtitles were grammatically broken in pretty much every line of text, so I edited all the subs to make them grammatically correct and more natural-sounding in English, and I fixed up some mistranslations I picked up as well. I removed the subtitles at the "diary" segment during the credits because they were incomplete, and my Japanese is still not good enough for me to have done it myself.
Tested the timing on NOP's release. Not sure about others.
