One correction to note: Miyafuji's military rank is Ensign, she had received a battlefield officers promotion.
In episodes 1 & 2, they show her rank as 軍医少尉 (surgeon ensign) and she’s referred to as such in the dialogue, so we use ensign or surgeon ensign as appropriate. In episode 3, she mentions that she and Shizuka are both ensigns, to which Shizuka replies, “That’s your rank as a doctor. In the 501st, you’re a petty officer!” Her rank doesn’t really come up in episodes 4 – 9. In episodes 10 & 11, she is referred to in dialogue as a 曹長 seemingly out of nowhere (which also surprised us), but it would seem that what Shizuka said is holding true, so as such we translate it as petty officer.
TL;DR – when they refer to her as 少尉 we translate it as ensign, when they refer to her as 曹長 we use petty officer.
